Your Role:
Working with clients in the community, you will:
- Deliver sound social work services such as assessments, investigations, diagnosis and treatment planning
- Provide excellent, evidence based social work interventions to our clients and their families or carers including comprehensive/Psychosocial assessment, advocacy, referrals, counselling and care plans
- Evaluate the effectiveness of treatment plans and modify as required
- Participate in care conferences and liaise with other health care professionals as needed
- Educate the general public and community workers in relation to our services and maintain waitlists
- Coordination of services
About You:
- Tertiary qualifications in Social Work and the relevant registration
- Experience in the provision of social work services with adults/elderly favourable
- Excellent communication skills
- First Aid & CPR certificate
- Queensland Provisional or above drivers licence
