Intake Coordinator Soldier On

YOUR NEW ROLE:

​The Intake Coordinator is the first point of contact for all participants and external stakeholders. The role is responsible for providing initial intake and triage for all incoming enquiries, as well as coordination and allocation of support for those registering as a participant at Soldier On.   

​​The position is integral to Soldier On and the Intake Coordinator will be expected to work closely with the Health and Wellbeing Director and the Psychology Practice Manager, as well as with the wider Soldier On team to appropriately link participants with the support that they need, both within the organisation and with relevant external providers.   

​ ​Dependant on experience, the Intake Coordinator will also provide brief counselling support to veterans and their families with the purpose of containing, re-directing, determining need and establishing goals for the purpose of allocation.   

Responsibilities include; 

1. Maintain a commitment to professional understanding of contemporary clinical best practice approaches, especially within a trauma focused and person-centred framework.  

2. Provide Case coordination support to Soldier On participants; 
​Coordinating intake appointments/triage via telephone, and online, and liaising with all internal stakeholders, including where relevant:  

  • Prioritise and respond to all incoming phone calls and email enquires,   
  • Conduct individual needs assessments, service planning, monitoring and evaluation,  
  • Linking participants in with other Soldier On services including education, employment, social connection programs and events, and psychological support,  
  • Allocate within a timely manner, ensuring accurate data entry for all intake systems at Soldier On and maintaining appropriate participant records and documentation, 
  • Determine the level of need of participants and support high needs participants through regular check in sessions,   
  • Monitor participant engagement, 
  • Contribute to the development and effective operation of the Intake process at Soldier On,  
  • Provide time limited counselling support to high needs participants with the purpose of containment, redirection and establishing goals to determine the support required,  
  • Maintain highest levels of confidentiality in all matters,  
  • Conduct risk assessments when required and respond to risk according to Soldier On Policy and Procedure and in accordance with state based legal requirements,  
  • Attend regular supervision to address risk and other related issues that emerge in participant intake and allocation,  
  • ​Work in collaboration with team members to provide best practice intake and allocation.  

Stakeholder Engagement; 

Social Connections; 

  • Attend Social Connections activities when requested (on occasion), 
  • Engage local participants and their families in a positive manner, when required. 

Community fundraising and grants; 

  • Provide local fundraising and grant opportunities to management and appropriate departments if required, 
  • Provide relevant information to management for applications and business cases. 

Media and Communication; 

  • Act as a spokesperson for Soldier On as a whole. 
  • Engage with local ESO’s, Stakeholders, businesses, and supporters for the purpose of increasing participant referrals, external referral pathways and funding streams, building a trusted reputation and awareness of the Soldier On programs and understanding local and market trends, 
  • Provide content to Management, Marketing and, Communications teams when requested. 

Governance; 

  • Keep effective and complete records,   
  • Adhere and manage Workplace Health & Safety processes, and   
  • Adhere to Soldier On governance processes, ensure State-based compliance.   

Other duties;  

  • Support the Practice Manager in the management of all Medicare billing, DVA billing, batching and electronic claims and ensure all provider details are current and correct in Halaxy.  

Qualifications, Skills, and Experience 

Essential 

  1. Relevant undergraduate tertiary qualifications in Social Work, Counselling or Psychology. 
  2. High levels of emotional intelligence and the ability to engage and work independently, as part of a multidisciplinary team and with other stakeholders.  
  3. Hold a valid Working with Children Check at all times and undergo Criminal Records Checks as required.  
  4. Interest in working with a specific but diverse population. 
  5. The ability to work autonomously and in a small-team environment. 
  6. Ability to embrace new programs and be flexible and adaptive. 
  7. Critical thinking and analytical skills.  
  8. Experience in maintaininghigh levels of confidentiality. 
  9. Excellent written and verbal communication skills. 
  10. Proactive problem-solving skills.  
  11. Excellent computer skills including all Microsoft office applications. 
  12. Willingness to learn and become highly proficient in Dynamics, Qualtrics and Halaxy.  

 Desired 

  1. Experience in an intake role that incorporated triage, client liaison, education, advocacy, assessment, referral and case management.  
  2. Experience in a national not for profit organisation. 
  3. Experience with Halaxy or other patient management software. 
  4. A broad understanding of the ADF and the experience of veterans and their family members. 

ABOUT YOU: 

The successful applicant will have demonstrable experience working effectively in teams, a good understanding of the challenges faced by veterans and their families and commitment to delivering positive and engaging services to the veteran community. 

You will have prior experience in case management, ideally in the employment space and be experienced in using CRM software
