Your role
As Reintegration Support Worker, you are responsible for delivering temporary residential accommodation and wraparound support services to program clients during the key transitional period following release from custody.
A key outcome of Post-Release Supported Accommodation services is to deliver service users through to secure longer-term suitable accommodation and maximise their capacity to be independent, stable and connected to appropriate social and community supports.

Key responsibilities
• Demonstrate an assertive and proactive approach to case management ensuring support is practical, respectful and comprehensive
• Provide wraparound support to service users in temporary accommodation including individualised support in relation to presenting issues pertaining to health, wellbeing and safety
• Undertake person-centred processes of assessment, planning, facilitation and advocacy in order to secure longer-term housing outcomes for service users
• Provide emotional and practical support, linking clients to referral agencies where appropriate
• Facilitate access to education, volunteering, employment or social and community engagement activities where possible to reduce social isolation and promote reintegration
• Connect service users with required resources, information and external services to assist in addressing presented issues and needs
• Document case management activities clearly and comprehensively in the required format, ensuring all follow up items are actioned and accurately recorded in relevant database/systems
• Ensure case management and support services are performed in accordance with set policies, procedures and legislative requirements
• Work in conjunction with Vinnies Housing Company in relation to any tenancy and property management matters which require action
• Maintain productive working relationships between property owners, lessors and sub-tenants
• Develop strong networks within the community and deliver advocacy for service users to establish access to support services and stable longer-term accommodation
• Develop professional working relationships within the Society, including committees and local conferences
• Follow up on internal/external queries promptly in a professional manner providing accurate details and information
• Ensure documentation and file(s) are maintained and up to date
• Follow up on queries and phone calls promptly and in a professional manner
• Provide reports and information as required to management or stakeholders
• Support a culture of safety, operational excellence and continuous improvement throughout the operations, with demonstrated year on year improvements in all areas
• Undertake other duties, which are assigned from time to time by your Manager
• Comply with all WHS duties and responsibilities as a worker in accordance with the document WHS Directive: WHS Roles and Responsibilities and the WHS Act 2011
• In all duties, comply with the behavioural expectations set out in the St Vincent de Paul Society Queensland Code of Conduct, and our policies and procedures
Qualifications and experience
• Tertiary level qualifications and/or equivalent demonstrated experience in community services, community development, corrective services, social work or a related field.
• Understanding of and commitment to the philosophy of the St Vincent de Paul Society Queensland and an ability to work in accordance with policies and procedures to promote the values and mission of the Society
