As a Mental Health and Disability Support Worker at Sunflower Services, your responsibilities will include engaging clients in community activities, maintaining accurate case notes, facilitating therapeutic conversations, and assisting clients in achieving their NDIS goals. You’ll work collaboratively within a multidisciplinary team to ensure the best outcomes for our clients while advocating for their rights and autonomy.
We currently have openings in both Supported Independent Living (SIL) and Individual Living Options (ILO) homes, providing varied and rewarding opportunities to make a meaningful difference in the lives of those we support.

- Light cleaning

- Implementing data for allied health professionals and data collection
- Uplifting, calming and positive conversations
- A commitment to helping Participants achieve their NDIS goals
- Following the Case Management Plan
- Following a Behavioural Support Plan
- Working within a multidisciplinary team of passionate individuals
- Cooking, meal preparations
- Transport
To succeed in this role, you’ll need to demonstrate experience in mental health, hold relevant certifications such as vaccination certificates, WWCC, police clearance, and NDIS Worker Screening Check, and possess a valid driver’s license with access to your own car and comprehensive car insurance.
