About the role
This position, for 4 or 5 days per week (negotiable) is aligned with the Australian Government’s YPIRAC Strategy and is contracted until December 2023.
Location is flexible across Adelaide metro region.
Working from home, you will be part of a vibrant and supportive national virtual team, supporting younger people (aged under 65 yrs.) who are at risk of, or who are living in Residential Aged Care. You will help people navigate State and Federal government systems including Disability (including NDIS), Health, Housing, and other relevant support systems to access age appropriate supports.
Regular travel around Adelaide metro region and surrounds is a requirement of this position.
Qualifications & experience
- Tertiary qualifications in an allied health, social work, nursing, or other relevant discipline
- At least 5 years of full time (or equivalent) complex case management experience
- Highly developed understanding of the needs and perspectives of people with disabilities and the impact on functional capacity.
- Critical reasoning skills and ability to translate this into succinct report writing for a range of different stakeholders
- Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
- A current driver’s licence and motor vehicle to use for work travel.
- Compliance with Pre-employment checks (including Working Screening Checks)
Tasks & responsibilities
- Work collaboratively with young people, their families, services providers, government agencies, community health services staff to coordinate the most effective options to meet preferred age-appropriate needs.
- Assess and actively monitor the effectiveness of selected strategies and supports
- Be involved in the design and implementation of internal quality assurance, complaints and continuous improvement systems
- Maintain effective and timely reporting of relevant client interactions.
- Contribute to the program’s research through the regular collection of research data and active participation in the Community of Practice forums.
