Position Overview: We are seeking a highly motivated and passionate Allied Health Professional to join our team and provide support to individuals under the National Disability Insurance Scheme (NDIS). As an Allied Health Professional at Hope and Care Community Services, you will have the opportunity to make a meaningful impact on the lives of people with disabilities and their families.
Key Responsibilities:
- Conduct comprehensive assessments of individuals’ needs and develop person-centered NDIS plans.
- Provide evidence-based therapeutic interventions and support services to clients.
- Collaborate with multidisciplinary teams to ensure holistic care and achieve positive outcomes.
- Monitor progress and adjust treatment plans as needed.
- Maintain accurate and up-to-date documentation in compliance with NDIS requirements.
- Advocate for clients and assist them in accessing appropriate services and resources.
- Educate clients, families, and caregivers on disability-related topics and self-management strategies.
- Stay informed about current trends and best practices in the field of disability support and NDIS.
- Undertake intake assessments and seeing through the participant onboarding process.
Qualifications:
- Bachelor’s or higher degree in a relevant Allied Health profession (e.g., Occupational Therapy, Speech Pathology, Psychology, Social Work).
- Registration with the appropriate professional board or association.
- Experience working with individuals with disabilities, preferably under the NDIS framework.
- Strong interpersonal and communication skills.
- Ability to work effectively within a multidisciplinary team.
- Empathy, patience, and a genuine commitment to improving the lives of individuals with disabilities.
