About the role
Palmerston Association have partnered with St Patrick’s Fremantle to provide the newly developed AOD Outreach Counsellor/Community Educator role based at St Patrick’s Community Support Centre in Fremantle. This role will provide assertive outreach support to Fremantle’s street present community while based in St Patrick’s Street to Home team. The role will involve collaboration with St Patrick’s other outreach teams and external service providers within the Fremantle area. The role will be managed by Palmerston’s Fremantle office, while daily support is provided by St Patrick’s Street to Home team leaders.
The successful applicant will have a well -established level of clinical knowledge, case conceptualisation and case management skills and a good understanding of AOD and mental health issues. as they will be expected to provide culturally secure counselling, education and support to individuals, families and the broader community. The successful applicant will require good networking skills and an excellent ability to communicate and develop relationships. This role is designed to help facilitate better relationships between service providers with a view to improving the treatment journey for people in the Fremantle community who are homeless or at risk of homelessness.
Pay Level is equivalent to Level 5 – Social, Community, Home Care and Disability Services Industry Award 2010.
