Your new company
This federal government agency provides a range of services and programs to provide support for veterans and their families. You will be working as part of an Australian wide team who address a wide spectrum of needs by providing support such as health and community care, rehabilitation, counselling, income support and education assistance. They do this via intake and assessments, telephone-based support and case management.
You will be joining an expanding team of passionate and highly professional specialist staff, within a supportive working environment which provides clinical supervision, professional development opportunities and peer and management support.
Your new role
As a Complex Case Manager (Clinical) you will be supporting veterans with identified vulnerability and a complexity of factors that require them to be supported navigating the various services that will benefit them. A key purpose of the role is to manage a case load of complex clients and to provide assessment and referral services via the phone to clients. You will deliver additional levels of expertise, support and clinical input by way of:
- Providing an escalation point for clients at serious risk
- Managing a limited case load of highly complex/high risk clients presenting with self-harm, suicidal ideations
- Consulting with stakeholders to provide clinical input, advice and expertise to inform the management of and response to urgent client escalations
- Supporting the delivery of reflective practice supervision
- Assisting in the identification and implementation of strategies to support high level clinical governance
- Completing and managing appropriate and required documentation for services provided
What you’ll need to succeed
- Qualifications and/or relevant registration with AHPRA as a Psychologist, RN (Mental Health), Occupational Therapist or Social Worker (AASW)
- Extensive mental health clinical experience
- Experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients in community mental health teams
- Excellent written and verbal communication skills, including capacity to liaise with and develop productive relationships with other organisations
- Experience managing clients in crisis including self-harm and suicidal ideation
