About the role
We are seeking a dedicated Assessment Intake Practitioner! You will work to assist Clinical Coordinators and clinical staff to coordinate referrals to the STAR Program. This requires interim support to clients who are awaiting treatment commencement through assessing client need, identifying treatment options and creating holistic treatment pathways.
Responsibilities include
- Procedures put in to enhance strong professional relationships between services evidenced by collaborative working relationships
- Bridging Support and Brief Interventions provided to clients who are awaiting assessment and treatment commencement
- Conduct initial assessment activities including competently assessing client risk and need
- Develop care plans with clear strategies to monitor outcomes and desired goals
- Referrals responded to in a flexible and timely manner
This is a fixed-term part-time position, 22.8 hours per week for 12 months based in Sunshine, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
You will have
- Relevant tertiary qualification in Social Work or related discipline
- Minimum qualification Diploma Community Services
- Relevant experience in a Social Service environment
- Relevant AOD qualifications and experience (highly desirable)
- AOD Skillset or willingness to undertake within first 12 months of employment
- A current Driver’s Licence
