About your new role:
Reporting to the Allied Health Manager, this role provides expert case practice advice, undertakes coaching and training, and supports the learning and development within the service. You will also undertake direct case practice, including development of Behaviour Support Plans for residents, post-incident support, direct therapeutic or behaviour intervention as well as reporting, and working collaboratively with families, carers, team members and external professionals by writing reports, making recommendations, writing and updating Behavioural Support Plans, Reporting Restrictive Practices and providing therapeutic care to residents as needed and desired.
Duties:
- To work in a multi/trans-disciplinary model with a range of allied health professionals to provide a holistic and integrated approach to support.
- Assist residents, their families, and team members, to identify individual needs through assessment and planning, developing meaningful goals and practical strategies to meet resident goals in their home and within our residential aged care facilities.
- Participate in the admission decision making process regarding residents with complex needs.
- Consult and liaise with residents, hospital and community staff, carers/family members, relevant agencies, and government departments, as appropriate to ensure holistic planning and support of the resident.
- Mentor and train colleagues to exercise independent clinical judgement concerning the election and application of principles, methods, and techniques of clinical assessment and/or therapy.
- Write Behaviour Support Plan’s, ensuring that all State and Federal legislative requirements are met for the use of Restrictive Practices.
- Plan, implement and review training and support plans for residents, team members and families in behaviour support and specialist topics as required.
- Assist to develop and deliver group psycho-educational and therapy programs, where appropriate, using established programs that are driven by empirically delivered clinical principles.
- Employ the principals of evidence-based practice to all Service Delivery.
Requirements:
- Relevant Tertiary Qualifications in Psychology, Social Work, Occupational Therapy, Disability or Social Services.
- Have a minimum of five years’ experience in the delivery supports to people with complex behaviour support needs.
- Have experience in dementia and developing plans to support responsive behaviours.
- Have explicit knowledge of Behaviour Support principles including functional behaviour assessment, development of behaviour support plans and implementation processes.
- Demonstrated ability to utilize data collection tools, collate information, analyze, and prepare reports for relevant stakeholders. Ability to work autonomously and as part of a team.
- High-level time management and caseload management skills.
- Ability to pass a National Police Check
- Willingness to obtain or provide proof of Up-to-date Influenza & Covid-19 Vaccinations
- Willingness to undergo a pre-employment medical
- Willingness to obtain National Disability Insurance Scheme (NDIS) check
- Willingness to obtain or provide proof of NDIS Worker Orientation Module
