We are seeking a Behaviour Support Practitioner for a well-established Disability Service Support Organisation located in the Western Suburbs. Flexible position, amazing workplace culture, NO KPI’s, offering $85k – $105k + super!
Our client is a leading disability service provider local to Southwest Sydney.
Position Overview:
To provide support, management, and assessments for clients/participants in the service of Behaviour support
- To observe and promote the clients/participants’ choice, independence, dignity, privacy, fulfilment and other rights.
- To create and maintain good professional relationships with participants, their family, friends and other stakeholders.
- To actively support and work together with other team members
- To adhere to all regulatory and statutory obligations and Jade Wellness Centre policies, procedures and guidelines
- To actively market Jade Wellness and promote a positive, personal and professional profile, ensuring the good reputation of Jade Wellness Centre at all times.
Roles and Responsibilities:
Clinical
- To provide Behaviour support to clients/participants with a wide range of needs and disabilities such as Autism, ADHD, developmental delay, global and intellectual delay and behavioural issues
- To create and implement Behaviour Support Management Plans for our clients/participants in and out of the centre.
- Provide implementation and training of the Behaviour Support Management Plans to all relevant stakeholders.
- To know and understand the needs and supports of the client/participant.
- To assist the client/participant to achieve their goals. For NDIS participants this includes (but not limited to) the goals they have set out in their NDIS Plan
- To encourage the independence, motivation and improvement of quality of life for clients/participants
- Regularly reporting to those in management positions at Jade Wellness Centre with any issues, progress or concerns
- To maintain good communication and develop effective strong rapport with clients/participants.
- Participate in multi-disciplinary team meetings and Case conferences as required.
- Develop a range of plans and reports including behaviour support plan and end of plan report for review and endorsement.
- Use behavioural, psychological and evidence-based theories to develop and implement program designs and interventions.
- Deliver training to implementing providers.
- Ability to positively engage with children, adolescents, adults and families.
- Be able to manage a range of clients/participants from Medicare, Private and NDIS
- Effectively communicate with clients/participants from a diverse range of backgrounds and cultures
- Assist in developing and carrying out group therapy sessions and workshops focused on Behaviour.
- Carry out appropriate assessments, such as Functional Behaviour Assessments with reports and provide recommendations for treatment/support.
- Record clinical notes after each session onto our client management system and write progress reports when required.
