The Organisation

This is a unique role as you will be working for two organisations taking a partnership approach to delivering a whole-of-family support to children and young people with varying support needs, living in the Brisbane Region.

The Program 

The program is funded by the Queensland government and is a free of charge service for families living in Brisbane South and Brisbane Southwest.

The program works with children 0-18 years who require a statutory out of home care placement and primary/respite Kinship Carers. They work collaboratively with the whole family including the parents and extended family to secure a stable and enduring kinship care placement while it is required. T

he Role

The role is a permanent, full-time position.

The Behaviour Support Practitioner is responsible for providing clinical services to children and young people in Statutory Out of Home Care – social and clinical assessments, therapeutic intervention, individual and group work. This includes development and review of clinical plans (therapeutic support plans, positive behaviour support plans), clinical assessments, social assessments, case management tasks, critical incident management, mentoring for kinship carers, foster carers and direct care staff, stakeholder engagement and afterhours on-call support.

Key Responsibilities

  • Develop, review and update Therapeutic Plans and Behaviour Support Plans for children and young people
  • Undertake Assessments for children and young people
  • Recruitment and assessment of kinship and foster carers
  • Undertake key case management tasks for children and young people including but not limited to reviewing key service documentation, facilitating and engaging informal and formal supports for children and young people
  • Participate in relevant stakeholder forums as required in the child or young person’s plans. Stakeholders may include government and non-government agencies who will provide key supports specific to the child and young person’s needs
  • Using the applicable Practice Model, service guidelines / frameworks, assist clients in achieving their goals as identified in relevant planning documentation developed in line with organisational procedures

To Be Successful

  • Bachelor Degree in Human Services or related field
  • Demonstrated skills and experience in working with children and families, particularly in assisting with the assessment, planning processes and support around the management of children with complex behaviours.
  • Current Paid Blue Card that does not expire within three months
  • Whooping Cough Vaccination (or be willing to get)
  • Current Open driver’s License 
  • LCS Check 

Covid Vaccination is not a mandatory requirement for this role.
