At Affirming Futures we believe in person-centred support, focusing on the child or adult and their family to achieve a holistic system of supports. We specialise in behavioural supports including complex behavioural supports and mental health. Our support services take place in the participant’s preferred environment, this includes schools, family home or community day option environments.
ABOUT THE POSITION (the position will involve…)
- Delivering behavioural therapy supports in accordance with behaviour support plans (oversight and support will be provided by our lead behaviour support practitioner).
- Training and support around developing Positive Behaviour Support Plans.
- Behavioural assessments of clients helping develop or following an already agreed treatment plan inline with the client’s NDIS goals.
- Providing education and support to caregivers and family members of clients.
- Evaluating results and progress of therapy on clients.
Skills required
- Compliant with state healthcare regulatory requirements
- Excellent interpersonal communication skills.
- Ability to work with clients with a lifelong disability
- Comfortable using a computer for various tasks
- Experience in working with disabilities
- Excellent knowledge of NDIS framework
- Excellent knowledge of the Disability Quality and Safeguard framework
- Excellent written and verbal communication skills
- Ability to build and work to KRA’s and KPI’s
- Excellent time management and organisational skills

Provision of Clinical Care
THERAPUTIC DELIVERY
- Implementing behaviour therapy programs through engaging directly with clients, parents/caregivers/support workers.
- Supporting implementing providers to deliver and implement strategies.
- Working directly with families and clients to provide supports as needed
- Attending meetings around clients when required to inform client program and progress as directed by Lead Behaviour Support Practitioner.
- Resources preparation or organisation of items/resources that inform therapeutic programming as directed by Lead Behaviour Support Practitioner.
- Collaborating with families to obtain feedback on implemented treatment plans and client’s progress.
- Engage effectively and respectfully with clients whom may exhibit behaviours of concern.
- Ensure client cases notes and data are consistently recorded within Affirming Futures database.
- Liaising closely with the Director of Behaviour Supports and Lead Behaviour Support Practitioner around client progress, PBSP progress and therapy sessions on a regular basis to ensure program fidelity and collaboration around ongoing behaviour therapy goals.
- Maintaining positive and collaborative working relationships with all allocated clients.
- Working collaboratively towards the provision of multi-disciplinary approach with other allied health professionals.
- Providing safe, effective, and efficient implementation of direct care in accordance with established policies, procedures and standards of care.
- Maintaining professional knowledge and continuous learning – ensuring up to date knowledge of practices and strategies within the professional field of behavioural support.
- Maintain confidentiality in line with Code of Conduct and comply with all Affirming Futures Policies and Procedures. Ensure time log entries are accurate and up to date.
