Care Coordinator Mental Health Integrated Complex Care Program Mentis Assist

JOB DESCRIPTION

Mentis Assist has provided community based mental health support to people living on the Mornington Peninsula and other localities for over 35 years.   Our Mental Health Integrated Complex Care Program (MHICC) works with people with severe and enduring mental illness who find it difficult to engage with mental health services.  The MHICC program is comprised of 4 service elements:

  • Clinical nursing services 
  • Family support and liaison 
  • Care coordination 
  • improving access to psychiatrist and psychological care 

Care Coordinators work with Mental Health Nurses and other clinical practitioners to ensure that clients are able to access the full range of services they require. 

We are looking for people to join our team at Narre Warren. If you fill the following requirements, we would love to hear from you:

  • Diploma of Mental Health or Community Services.  Applicants with a psychology, social work, or occupational therapy degree and relevant experience will be highly regarded 
  • 3 year’s experience working in a mental health or community services setting 
  • Current practical knowledge of evidence based theories of the individual in society 
  • Demonstrated skills and experience in focused psychological strategies   
  • Current drivers licence 
  • Complete COVID vaccination (including boosters)
