YOUR NEW ROLE
Care Coordinators work with clients with a chronic condition and/or complex needs along with member of the multi-disciplinary team to ensure that the clients have the most appropriate services available when required. Care coordination assists in reducing duplication, delays and conflicting information. This work is applied under an early intervention in chronic disease (EICD) management framework and is underpinned by the care for people with chronic conditions guidelines.
Management of clients is based on level 3 of the Chronic Illness Care continuum.
Care coordinators will achieve this by providing:
• Client centred care that is culturally responsive
• Client-focused broad multidisciplinary assessment and appropriate care planning
• Supports a multi-disciplinary team approach to care
• Build client self-management capacity
• Contact and support for the client when under the care of Care Coordination
• Support the client and significant others to determine their preferred care options and most appropriate pathways
• Early intervention to minimise adverse events and promote ongoing health and wellbeing
This service is underpinned by the Wagner Chronic Care Model that helps identify system changes necessary to provide an integrated and proactive service that improves the coordination of care for clients.
YOU WILL BE RESPONSIBLE FOR
• Provide a comprehensive multi-disciplinary health and/or wellness assessment to clients
• Assist in identifying the needs of the client and carer with health changes and their capacity for self-management
• Implementing motivational interviewing techniques and health behaviour change strategies to encourage clients to be active participants in their care
• Provide a point of contact and support for clients and significant others throughout their episode of care until discharge
• Facilitate multi-disciplinary and coordinated approaches to care through development of a goal directed care plan and initiate case conference where required
• Work collaboratively with internal and external stakeholders to support clients’ including working with the immediate multi-disciplinary team, other internal Merri Health services and external supports such as GPs, other relevant health professionals.
• Support program staff in facilitating multi-disciplinary student placements
• Support and supervise the practice of Allied Health Assistant staff (as required)
• Play a key role in the Integrated Diabetes Clinic and Chronic Pain Service within Merri Health
TO SUCCEED IN THE ROLE YOU WILL HAVE
• Tertiary qualification in one of the following: Allied Health Profession (Dietetics, Occupational Therapy, Physiotherapy, Psychology, Social Work, Speech Pathology) or Nursing (RN Division 1 qualification in Nursing)
• Demonstrated experience in client-centred care through holistic assessment, care planning, multi-disciplinary case conferencing and discharge planning
• Knowledge and experience of working with clients with complex needs and/or chronic disease management (including interest in working with diabetes and chronic pain programs)
• Highly developed written and verbal communication skills
• Demonstrated experience in data collection and analysis
• Experience with clients who have chronic and complex health needs with a particular interest in diabetes and chronic pain
• Experience working in a community health/community rehabilitation/outpatient setting
