JOB DESCRIPTION

  • Max term contract until 30th June 2024 (with possible extension)
  • Part-time position (0.6 FTE) 6 days a fortnight

ForWhen provides new and expecting parents—mums, dads, and guardians—with a caring, supportive mental health navigation service, to guide them and their family throughout your perinatal journey, from conception, up until their child is 12 months old.

The team connect parents who are struggling to navigate the complex waters of pregnancy and new parenthood to the critical mental health services in their state.

About the Position

We are seeking a part-time Care Navigator, to be based at Ngala, Kensington.

Working with a child-centred, strength based and collaborative approach, your key responsibilities will include:

  • Identifying state-wide PIMH support services and referral pathways.
  • Building productive relationships and stakeholder engagement with key individuals and groups such as clients, families, National PIMH ForWhen Team (including Navigators in other states/territories), broader Ngala staff, PIMH support service providers.
  • Connecting and care specific tasks including but not limited to: intake; screening; and triaging and allocation of families to local services.
  • Ensuring the effective co-ordination and integration of services; timely, accurate and quality record keeping; and project monitoring.

About you

You will possess:

  • Bachelor or Diploma in Social Work, Nursing or Psychology.
  • Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA); Nursing or eligibility for membership with the peak professional association (Social Workers).
  • Knowledge and understanding of mental health issues in antenatal, birth and the postnatal period (including maternal and infant health, parenting).
  • Knowledge and awareness of current and emerging issues and trends in perinatal infant mental health, including barriers to accessing support services.
  • Demonstrated experience of collaborative person-centred care and treatment planning and working with families; including an awareness of barriers to engagement.
  • Demonstrated ability to work in a multidisciplinary team and liaise with other health professionals.
  • Ability to undertake a comprehensive perinatal-specific biopsychosocial assessment and use this to inform referral pathways and plan treatment as required in the perinatal context.

If successful, you will need to possess a WA driver’s licence, satisfactory National Police Clearance obtained within six months, and current WA Working with Children’s Check.
