  • Salary: $43688 to $45909 + Super + Salary Packaging

Your Opportunity

The Integrated Carer Support Service links carers with supports and services to help carers manage their daily challenges, address their stress and maintain wellbeing, and plan for the future. Supports include early intervention, prevention and skill building to help carers stay well.

The Carer Coach fulfils an important role within the Carer Gateway program by facilitating the Carer Coaching sessions for carers who have completed registration and an assessment with the Carer Support Planner and together they have decided Coaching would be beneficial to the carer.

Key responsibilities will include:

  • Facilitate one-on-one coaching to carers, to achieve the program’s required KPI’s.
  • Refer carers to in-house and/or external Carer Counsellors, Peer Support Workers, and Planners as appropriate.
  • Develop strong relationships with key external stakeholders.
  • Complete all necessary administrative tasks associated with care coordination and program activities including accurate database entry.

Requirements for Success

  • Qualifications and training in the Community Services/Youth/Health/Welfare/ Social Services and/or Education fields or a minimum of three years’ relevant industry
  • Demonstrated knowledge and understanding of the complex challenges faced by Carers of people with a disability, chronic illness, mental health condition and the aged.
  • The ability to empower carers to understand their needs/goals and to make decisions and actions independently.
  • High level computer skills including client management systems, the Microsoft Office suite of products and the ability to learn new systems when required.
  • Current NSW Driver’s License.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
