We are looking for more than experience, we are looking for the “right attitude”. This is a full-time position and in this role, you will be responsible for delivering a superior standard of clinical care and management to our community clients. You will oversee all our Home Care Package clients as well as our clients who have Commonwealth Home Support Packages.

What your days will look like:
- Monitoring care and maintaining ongoing communication regarding the health status of clients
- Reporting changes on the health and wellbeing of our clients
- Liaising with Rostering to negotiate changes to client services based on client needs and preferences
- Collaborate with Community Registered Nurses to facilitate planned nursing care to community clients.
- Be flexible to travel to all clients across Port Stephens in order to visit your clients in person on a regular basis
