Case Manager Empowering Minds and Development

JOB DESCRIPTION

In the role of NDIS Case Manager you will assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan.

You will also be required to establish strong rapport and working relationships with a range of stakeholders including People with Disability & their family members, NDIS Local Area Coordinators, NDIA Planners, internal and external service providers.

Key responsibilities are:

  • Engage in evidence-based best practice case formulation, assessment, reporting, and interventions for clients identified as complex needs.
  • Manage an ongoing and long-term case load of clients, including achievement of billable hours per week. 
  • Undertake informed referrals to appropriate allied health professionals and/or service providers.
  • Initiate and maintain productive relationships with clients, stakeholders, informal supports, and service providers who may present with differing priorities.
  • Manage client’s NDIS plan to ensure services are being delivered within their allocated budget.
  • Develop and provide reports on outcomes and success indicators, within the required timeframes. 
  • Develop strategies which will address the identified needs of the client in consultation with their representative/s where appropriate, in accordance with funding guidelines.
  • Support clients and their stakeholders, to understand the supports in their plan, navigating NDIS processes, identifying and accessing mainstream, community and funded supports.
  • Manage accurate record keeping and the security and confidentiality of records and information in accordance with Information Privacy Standards.

To be successful in this role, the ideal candidate will be someone who can demonstrate the following:

  • Knowledge and/or the ability to rapidly gain an understanding of NDIS, disability and community services sector.
  • Experience in a case management role and or support coordinator role.
  • Exceptional customer service, negotiation and influencing skills.
  • Excellent ability to connect and build rapport with multiple stakeholders.
  • Strong interpersonal skills and high levels of resilience.
  • Excellent verbal and written communication skills.

 Desirable

  • Degree in Social Work, Psychology, Counselling, Criminology or related field
