This position is a contract in line with QPP’s Service Agreement ending 30 June 2026.

Queensland Positive People (QPP) are seeking a passionately motivated case manager to provide client-centred support to people living with HIV who are experiencing barriers and impacts to HIV care and treatment.

This position covers Brisbane. We are looking for a case manager in either a Peer Treatment Facilitator role (if eligible), ORTreatment Support Facilitator role

Case Manager – Treatment Support Facilitator role

The treatment support case management role, with a strong focus on client empowerment, provides a variety of experiences and learning opportunities for staff. The role requires skills and experience in providing complex case management support for those experiencing challenges and issues such as mental health, finances, housing, and employment as well as comprehensive project management, time management, facilitation and event coordination skills. The TSF will also build tailored action plans in collaboration with clients to overcome identified barriers and address the social determinants of health that impact treatment uptake, adherence, and retention in care.  

Case Manager – Peer Treatment Facilitator role

This role requires the use of the lived experience of HIV and the application of case management and self-management framework, as such is required to be filled by a person living with HIV who is comfortable sharing their experience with clients and the community. The PTF will assess client circumstances and needs holistically and provide peer education and practical assistance to navigate the health system. The PTF will also build tailored action plans in collaboration with clients to overcome identified barriers and address the social determinants of health that impact treatment uptake, adherence, and retention in care. The role requires skills and experience in providing complex case management support for those experiencing challenges and issues such as mental health, finances, housing, and employment as well as comprehensive project management, time management, facilitation and event coordination skills.

  • Hold a current QLD Working with Children Check (Blue Card)
  • Provide a national police check of 6 months currency
  • A valid driver’s license with no restrictions
