Case Manager – Role Summary
The Case Manager will work with a caseload of clients/participants in the NDIS or OOHC space, meeting the individual needs of those within your case load. You’ll provide leadership to other youth and disability support workers, ensure your team(s) deliver the most exceptional support possible, and are working towards identified goals. The Case Manager will review shift reports and incident reports and provide guidance or support to other frontline workers when required.
As a Case Manager, you will regularly roster frontline workers to shifts within your case load, ensuring the right skills and experience exist to effectively support your clients/participants. You’ll be required to maintain industry and company compliance targets that will contribute to quality indicators and accreditation initiatives.
The Case Manager will actively promote ICCare and liaise with external stakeholders in different government and non-government organisations ensuring clear and effective communication. A Case Manager will participate in remote after hours on-call support to our frontline workers and will receive additional remuneration for your participation in the on-call roster.
What you need to be successful
- As a minimum, diploma in community services, or a related field.
- Current drivers licence and first aid certificate
- Proven experience using IT platforms and the ability to quickly learn new technology
- Evidence of a paid working with children check, clearing you to work with children and young people
- Experience in case work or case management or considerable frontline experience
