About the Role:
Anglicare has an excellent career development opportunity for a Case Manager based in Parramatta! In this role, you will provide case management services to individuals with mental health needs through support, advocacy and internal and external referral pathways with local service providers. You will be assisting individuals to gain the skills and confidence necessary to manage their mental illness and its effects within a recovery framework.
This is a maximum term, full-time position for 10 months and is classified under the SCHADS Award.
Key responsibilities (not limited to):
- Ensure the best possible outcome for participants is achieved by advocating for participants, establishing strong network links within local area in order to provide strong external referral pathways and developing strong partnerships with the church as a support resource
- Ensure the service is well known in the community by developing resources that promote and advertise the service to participants, local forums and other community organisations
- Ensure individual needs of participants are met by providing a recovery plan that is developed with the participant, is clearly defined and with achievable outcomes
- Ensure a full participant caseload is maintained by working within funding guideline and program targets
About you:
- Relevant tertiary qualifications or studying towards which may include Social Welfare, Social Work, Health or Social Sciences Nursing, Psychology
- Unrestricted NSW Drivers licence and own vehicle
- Experience working with people who have a mental illness with functional limitations
- Excellent written and verbal communication skills
- Current NSW Working with Children’s Check
- Current NDIS Check
- COVID-19 vaccination is a mandatory requirement unless assessed exempt
- Experience coordinating groups will be an added advantage
