Due to continued growth, we are currently on the lookout for a driven and energetic Case Manager to join our Newcastle team!

About You – What you need to succeed 

  • Previous aged care case management experience especially Home-Care Packages
  • Demonstrated knowledge of the Aged Care funding model and experience ensuring all reporting and administrative matters
  • A minimum Certificate IV in Aged Care or equivalent – other health professionals including RN’s and Allied Health professionals are encouraged to apply. 
  • Strong service coordination, organisation skills and experience in Complex Case Management and supporting families in crisis, would be highly desirable
  • Experience in a result driven environment.
  • A passion for exceptional customer service, negotiation and influencing skills
  • Demonstrated ability to balance conflicting priorities and ability to connect with multiple stakeholders 
  • Confidence and proficiency using a customer database or online CRM tool.
  • Current NSW driver’s license and willingness to drive for work purposes 
  • Own reliable vehicle with comprehensive insurance. 
  • A current Influenza and COVID-19 Vaccinations

Developing and maintaining good relationships with relevant staff to ensure optimum outcomes for consumers.

Maintain your own professional development in line with the requirements of the role in accordance with current standards. 

Ensure compliance and fulfilling the requirements of Aged Care Quality Standards.
