JOB DESCRIPTION

Case Manager

Community Transitions deliver innovative and proactive services and meaningful interventions to meet clients mandated requirements, to enrich their lives and enable independence. The role is based at Port Adelaide and works within a team of experienced and dedicated case managers providing outreach support to individuals on home detention or other community-based orders. 

You will have significant active outreach experience, be a confident and capable case manager, able to undertake assessments, develop and manage support plans, have excellent case noting (ISBAR), and make best use of your networks to proactively access services and supports.

The role is Level 5 within the SCHADS Award which is currently $93,858.96 per annum (pro rata). Above Award leave entitlements, which are 22 days annual and 12 days sick leave per annum (pro rata), and salary sacrifice is applicable.

  • Proven outreach case management experience 
  • Effective time management skills
  • Tertiary qualifications in Social Work or related discipline
