We are seeking a highly motivated Case Manager and program support professional.  Working with our highly experienced team, the successful candidate will play a pivotal role in helping to provide key support for fathers and their children where it’s needed most.

The Role: 

This position is part of a Community Fathering team and is responsible for providing Case Management and program support to clients.  The purpose of the role is to assist and empower clients ie men who are fathers or father figures.  This is done by providing Case Management support in accordance with the National Standards of Case Management and by providing a variety of support options and resources to ensure clients are enabled to progress towards meeting their goals.   TFP deliver these services in a client-centred, trauma informed, strengths-based way utilising up to date research.  

This role will also involve engaging schools and other community organisations in order to provide support and engagement of fathers and father figures in a multitude of different settings.

 

Key Responsibilities:

  1. Build rapport – With fathers and father figures while maintaining professional boundaries.
  2. Complete assessments – Perform assessments to tailor support through both formal and informal methods: observations, family history and stakeholder information.
  3. Develop – A solid case plan with your clients in accordance with their assessment, goals and services involved.
  4. Case management – Deliver solid case management for clients to develop intervention strategies to engage participants to achieve both program and individual goals.
  5. Ensure – Input all information contemporaneously into data systems to record and maintain participants personal information in accordance with service agreement/s, legislation and program policies and procedures.
  6. Contribute – Further develop and maintain relevant community partnerships via networking, and actively promoting other stakeholders through referrals and promotion of programs.
  7. Schools and services – Build relationships and provide support to local schools and services in order enhance our local community.  

 

About you:

  • You have 5+ years of case management experience (working with families preferred)
  • You have excellent and concise written and verbal communication skills
  • You are results and outcome oriented and experienced at building networks
  • You have excellent interpersonal and rapport building skills 
  • You are passionate about supporting fathers and families to improve wellbeing 

 

To be considered for this position you will have:

  • Tertiary level qualifications in Social Work, Psychology, Counselling, Social Welfare or a related discipline.
  • 2+ years of supporting children and families.
  • Demonstrated Case Management experience with at risk adults, preferably men, children and their families.
  • Demonstrated ability to provide support in a group setting.
  • Demonstrated understanding of child protection issues and an ability to conduct risk and family assessments using a strengths-based approach.
  • Demonstrated ability to work with men and families in crisis and work in partnership with them to achieve their goals, using a variety of evidence-based intervention strategies.
  • Demonstrated experience in supporting Aboriginal families using culturally safe and sensitive interventions.
  • Commitment to ongoing training and development to ensure a broad range of evidence-based models as determined necessary by the program.
  • Current Working with Children Check.
  • Current driving license.
