About You 

To be successful in this role you must be compassionate and have an awareness of the impacts of age or disability on the physical, social, and psychological capacity of clients. You will have excellent written and verbal communication skills and the ability to work both autonomously and in a team environment.

The Role

In this role, you will be providing one-on-one support to clients from a range of diverse backgrounds, working collaboratively to help meet their goals and needs.

Responsibilities

  • Work with clients to achieve their NDIS goals.
  • Show a high level of professionalism and empathy for the unique needs of clients with complex disabilities.
  • Provide ongoing mental health recovery coordination using a coaching approach to assist clients in capacity and independence building.
  • Work in close communication and cooperation with management regarding all supports provided to clients.
  • Liaise with support workers and administrative staff regarding changes to a client’s care plan and support needs. 
  • Ensure accurate and professional progress notes are recorded for each client.
  • Conduct incremental reviews of client progress through psychometric analysis.
  • Conduct incremental plan reviews to ensure client funding allocations accurately meet their needs and care requirements.
  • Engage with clients, their supports, and key stakeholders.
  • Build and maintain up-to-date knowledge of local services and activity options.
  • Support client engagement in training programs and encourage the retention of clients and their active participation in programs and activities.
  • Maintaining trust and positive relationships with clients,
  • Show a clear dedication to maintaining personal integrity, company image, client confidentiality, and best practice.

Must haves: 

  • Relevant qualification in Psychology, Counselling, Community Services, or similar.
  • Experience working with diverse clients.
  • Excellent communication skills and use of the English language both verbal and written.
  • National Police Clearance.
  • NDIS Screening Check, (or willingness to apply).
  • Experience with Microsoft Office Suite (Word, Excel, One Note, etc.).

Would like: 

  • 12 Months of healthcare industry experience (preferably working with people with disability).
  • Previous experience in Support Coordination.
  • First Aid and CPR certification.
