About the role
Full time – 38 hours per week
The Case Manager works with clients accessing QuIHN Therapeutic Services, and their counsellors in a case management capacity, including travelling to identified outreach areas.
Key responsibilities include:
- Provide an outreach case management program
- Conduct home visits within outreach areas in accordance with established policy and procedures
- Support clients through the stages of change process with relapse prevention goals and/or harm minimisation goals
- Support clients, and significant others, with welfare needs, housing, education and employment, through active referral pathways as part of a case management plan
What we’re looking for
Someone who is passionate about their work and the health outcomes of both clients and the broader community, and can demonstrate:
- Tertiary qualified in a relevant field i.e. Bachelor of Counselling, Social Work, Social Science, Psychology or Human Services
- Ability to engage and work with vulnerable and complex individuals and/or families affected by substance use and other related concerns
- Proficient computer skills – Microsoft Office Suite, databases and other client management systems
- Available to work between the hours of 8:30am – 8:00pm
- Willing and able to undertake outreach, at a minimum of two days per week or as directed, at another service or appropriate location within a community of identified need in relevant locations
- Current Queensland driver’s licence (or equivalent)
