A Case Management position specifically for the demographic of under 65’s-year-olds. It is an outreach program which assists people under the age of 65, with diverse and complex needs and links them to relevant community services from an intensive case management framework.
Clients are seen in their own home, within the local catchments of Port Phillip and Stonington. In consultation with the client, key goals are identified using a collaborative, holistic, approach with the aim of maintaining client independence, stability, and support.
The target client group is often marginalized and isolated. Frequently they present with a broad range of issues. A case load relative to the EFT is managed by the worker. They have a fundamental role in advocacy and service coordination, this may include (but not limited to) testing National Disability Insurance Scheme (NDIS) eligibility, issues/needs related to Centrelink, making referrals for health, housing, Alcohol and other Drugs (AOD), counselling, mental health, disability, allied health, nursing, and numerous other service pathways.
The case manager links the client to such services whilst continually and comprehensively reviewing their plan of care.
Ideally, you will possess:
- A professional qualification in Social Work (minimum of 2 years post graduation experience).
- Experience in diverse case management/social work experience, resulting in comprehensive service coordination.
- Demonstrated ability to network and utilize referral pathways i.e. NDIS, AOD, Mental Health, Centrelink and so forth.
- Excellent knowledge of Complex client care in marginalized communities.
- A clear understanding of professional accountability in managing a client caseload.
