We are new to Adelaide, coming from a well established and supportive base in Newcastle.

Role Overview:

As a Case Manager at Sunflower Services, you will play a vital role in operationalising support for your portfolio of NDIS participants. You will be responsible for managing high-quality, individualised services that promote choice and control for our participants. Leading by example, you will coach and mentor your support team, ensuring they provide excellent, high-quality services. Collaboration with participants and their families to achieve outstanding outcomes is at the heart of this role.

What We’re Looking For:

We are looking for an experienced and well-organised Trauma-Informed Case Manager who has the knowledge and skill set of the NDIS and can share and pass on knowledge derived from previous positions. A background in Mental Health and Psychosocial Disability is a must, and we only encourage applicants to apply if they have previously managed a case load of participants. Our ideal candidate is someone who excels at building relationships with participants, their families, support workers, peers, and other stakeholders. You are tenacious and thrive on solving challenges, making a meaningful difference in the lives of our participants. You are eager to get involved and passionate about the work you do. This role will include building a case load, bringing on new participants and hiring support workers. 

Excellent verbal and written communication is essential. And an acceptance and willingness to help grow the business and its Adelaide presence. 

Requirements:

To succeed in this role, you must clear all state-based probity checks, including a NDIS Worker Screening Check, and Working with Children Check. A valid SA Driver’s License and evidence of COVID vaccination are also required.

Key Responsibilities:

Ensure support services align with participants’ needs and goals.

Foster effective communication with participants, families, support teams, and stakeholders.

Cultivate a high-performance culture focused on delivering outstanding service and quality outcomes.

Lead and manage teams to achieve participants’ goals.

Provide 1:1 case management with participants with disabilities.

Provide 1:1 mentoring, support, and management to support workers. This includes providing relevant feedback and building team skills and strategies. 

Develop Case Management Support Plans and Risk Assessments. 

Provide leadership for high-quality life outcomes.

Manage staff rosters for efficient utilization.

Offer management support and practice leadership.

Promote participants’ directed support exceeding NDIS standards.

Assist in the marketing and business development of Sunflower Services. 

Drive team efficiency and implement new ways of working.

Implement Behavior Support Plans (BSP) in collaboration with allied health teams.

Ensure effective operational management within policy and procedural guidelines.

Establish and maintain collaborative relationships with key internal and external stakeholders.

Overall responsibility for performance within the assigned area.

Networking with other providers and NDIS affiliates

Assist with the hiring of Support Workers

Additional Requirements:

Strong background in Trauma-informed care and the NDIS Disability Services. 

Strong leadership and administrative skills.

Have strong cultural competency knowledge. 

Ability to multitask effectively.

Integrity, honesty, and respect in all aspects of work.

Current open Driver’s License and reliable, registered vehicle.

Minimum of a Diploma of Case Management, Counselling, Mental Health or Community Services. A relevant degree is preferable not not essential if the experience is valid.
