The Organisation:
TransitCare is a not for profit organisation that provides socially inclusive, safe and reliable transport and support services to assist socially isolated people and people with disabilities to live independently in their own homes and participate in their communities. Main services provided are social and medical transport and activities for clients living in the Brisbane South, Logan and Redlands areas. TransitCare is proud of our recent completion of our Reflect Level RAP. We also manage the delivery of similar services in our Townsville, Cairns and Gold Coast branches. The Role:
We are seeking a Case Manager to join our Home Care Package team.
The role will include duties such as;
- Liaising with clients as well as their families on a regular basis to assess client satisfaction and address concerns as required
- Conducting client assessments
- Collaborating with internal and external stakeholders to ensure seamless service delivery
- Monitoring service delivery and address changes to client needs
- Maintaining up to date client records
- Generating regular reports for management and quality improvement purposes
- Participating in team meetings
The Person:
You will need to have completed a Certificate in Community Services or equivalent.
You will need a current drivers license and be able to travel to the office at Loganlea as well as to clients homes within the Brisbane South and Logan areas. You will need to have the ability to obtain a Workers Screening card and a Blue Card as well as a Federal Police Check.
Previous experience as a Case Manager is highly desirable.
Previous experience within Aged Care is preferred.
