Base pay scale: $45.00 – $48.25 per hour (includes 25% casual loading)

Located in Nambour on the beautiful Sunshine Coast, our Graceville Centre provides support to individuals living with a disability and/or mental illness. All Graceville services are conducted by working with individuals in a strength based and recovery focused framework. Graceville Centre operates eight programs that provide support to clients either on site or in their own homes. We are a growing service and regularly have internal opportunities for career growth.

As a Case Manager with a Lived Experience of Recovery you will provide overarching case coordination and case management support to clients living with mental health concerns and/or a disability across various Graceville Centre programs. The role is responsible for providing support to help individuals increase their independence, maintain their wellness and ultimately improve the quality of their life.

In addition to the case management objectives, this role will also utilise the individuals unique experience of living with a mental illness to assist clients, their stakeholders, and Lutheran Services to better develop an understanding of the recovery framework and ultimately achieve better outcomes for individuals.

Responsibilities will include:

· Utilise and apply knowledge of lived experience practice within day to day interactions with clients, their families, stakeholders and other staff

· Support management in educating the staffing group on the principles and application of lived experience practice and its wide-ranging benefits

· Provide advice to management to incorporate lived experience in the development of organisational policies and procedures

· One on one client facing work including medication assistance, personal care, domestic assistance and assisting clients in reaching their personal goals

· Involvement in implementing and reviewing case management plans and liaising with clients and stakeholders

· Utilise strengths-based practice in the development and implementation of client’s individual support plans;

· Maintain an environment that empowers service users and promotes positive behaviours, positive learning, skills development and independence

· Minimum availability to work 2 sleepover shifts per week (if required)

 

About you:

The successful applicant will:

· Identify as having a lived experience of recovery;

· Personal experience as a mental health consumer;

· Understanding of lived experience concepts and contemporary research;

· Peer work or Lived Experience qualifications or training delivered by a professionally recognised organisation;

• Effective and professional interpersonal and communication skills

• Accurate and timely decision-making skills

• Ability to work in a fast-paced environment

• Must be comfortable working both autonomously and as part of a team

• Basic understanding of Outlook, Excel, Word and Accessing apps on your mobile phone.

• A positive attitude and willingness to learn on the job
