Yulu-Burri-Ba is currently seeking a highly motivated Case Manager to join our Social Health Team!

This position is to provide culturally appropriate clinic and community based supports to Aboriginal and Torres Strait Islander clients who experience complex social and mental health conditions and to work effectively with other social and health practitioners to identify and access appropriate services to meet their needs.
The position will be responsible for:
· Coordinate and facilitate effective support structures, including external agencies in order to meet the complexity of client needs i.e. liaising with Centrelink, Dept of Child Safety, Dept of Housing Service etc;
· Intake and assessment of referrals to aid appropriate allocation of services i.e psychologist, social worker, family wellbeing service, AOD counsellor etc;
· Assist in the delivery of counselling services through a social work framework that focusses on solutions focused strategies, brief interventions, education, harm minimisation techniques, crisis intervention and general support relevant to client’s mental health needs;
· Manage a case load of clients with complex needs and identified issues;
· Empower and foster independence and interdependence through participation, and access to appropriate resources, opportunities and services within the community;
Essential
· Demonstrated ability to work with vulnerable client groups, including the provision of social supports and addressing general client needs;
· Demonstrated knowledge of Aboriginal Community Controlled Health services and the issues facing these organizations;
· Knowledge or experience in effective case management;
· Knowledge and experience in effective counselling techniques
· Ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people;
· Demonstrated ability to work as part of an inter-professional team including knowledge and understanding of the roles and contributions of other professionals;
· Demonstrated effective verbal and written skills with particular emphasis on interpersonal communication skills, confidentiality and record keeping
· Ability to work across all three YBB clinics (North Stradbroke Island, Capalaba, Wynnum)
Qualifications/Experience
· A professional qualification in Social Work, Human Services, Nursing, with registration in relevant body or other relevant qualification or a Diploma in Counselling or related field with extensive relevant experience
· Demonstrated practical experience working with vulnerable clients associated with drug and alcohol or mental health conditions
· Desirable Mental Health Accreditation
Practical Requirements
· Current Cardio Pulmonary Resuscitation Certificate (CPR)
· Current First Aid Certificate (If applicable for position)
· Current “C” Class Driver’s Licence valid in Queensland
· Eligibility for a Suitability Card for Child Related Employment (Blue Card registration)
· Proof of immunisation Status – or willingness to obtain as a matter of priority
