About the roles

Targeted Care Package (TCP) Case Manager:  The program works with young people who are supported by Targeted Care Packages, an initiative of the Department of Families, Fairness and Housing (DFFH).  The purpose of these packages is to enable the transition of eligible children and young people from residential care to more appropriate care arrangements where their care needs will be better met.  Alternative placements that can be considered as part of the package include placements with parents, family or friends, a foster carer, independent or semi-independent living, or other forms of care.

Case Coordinator – Residential Care: will work with young people across all of Junction’s Residential Care homes in Gippsland to offer support and development across the domains of system connections, community connections for young people and child protection case management support. This role will undertake an assertive outreach approach to support young people in residential care by undertaking tasks which will assist them in developing and achieving their goals.
