Case Worker – Mental Health Momentum Collective


Momentum Collective is a registered charity and not-for-profit community services organisation creating real social change and inclusive opportunities. We have a proud 40 year track record operating in a suite of programs in disability, mental health, homelessness, social and affordable housing, family & domestic violence, child family and community services (rearrange to put mental health and child family first)
We believe in a shared or collective responsibility to help close these gaps and are looking for can do, positive people, to join our community movement.
In this role you will provide early intervention support for vulnerable families with children and young people who are showing early signs of, or are at risk of developing a mental illness. You will also Support children and young people to improve their wellbeing and enable them to better participate in their communities and reach their full potential.

  • Tertiary qualification in a community service, social science or related field and/or specialised skills & experience sufficient to perform at this level
  • Demonstrated experience in case planning and coordination of people with complex support needs
  • Experience working with children, young people and families
  • Demonstrated experience and or understanding of Trauma Informed Care
  • Understanding and experience of working with Aboriginal people and their communities
  • Working with Children Check
  • Satisfactory Criminal record check
  • Current Driver’s Licence
