Your new role:
Vital and rewarding role providing high quality case management, group work and general drug and alcohol treatment and other support to clients of the Freeman House programs. You will be joining a small but dedicated team who are passionate about helping people to change their lives.
Responsibilities include:
- Provide drug and alcohol case management and treatment support services to clients of Freeman House in accordance with the Society’s AOD Model of Care.
- Assist and support clients transitioning from withdrawal to internal and external services including residential rehabilitation, partnering treatment programs, and community organisations.
- Work in partnership with other members of Freeman House staff including other Case Workers, Nurses, Counsellor and Support Workers in order to ensure quality client outcomes.
- Provide support to clients of the Freeman House services including ad hoc psycho social support, conflict resolution or any other support required.
- Support and engage with family and significant others as appropriate, including through referral to external services.
- Facilitate groups within the Freeman House group program schedule.
- Maintaining accurate and up-to-date client records and case notes
To be successful in this role, you will need:
- Minimum TAFE Certificate IV or currently undertaking tertiary study, in welfare or community services related field.
- Demonstrated experience in managing a case load and case planning, including with persons with complex needs
- Demonstrated experience in providing treatment approaches designed to support clients in their treatment journey.
- Demonstrated ability to conduct self in a professional, ethical and non-judgmental manner.
- Knowledge of, and an ability to access, a range of relevant community resources
- Basic computer literacy skills including experience in computerised data collection
- A valid NSW drivers’ licence.
