Your Opportunity

Provide structured Alcohol and other drug group programs and individual health and social interventions and support for young people aged 12 -24 years living in the community.

Your key responsibilities will be to:

  • Respond to referrals of clients to the service from internal and external support services and conduct formal assessments and collaborative care planning.
  • Deliver group-based interventions to young people
  • Provide ongoing case management sessions with clients and review progression
  • Ensure that all required internal and external client paperwork is completed and copies kept on file

Requirements for Success

  • Relevant Qualifications and/or relevant industry experience
  • Understanding of issues impacting young people and addiction or experience in that field
  • A working knowledge of Case Management and the delivery of group based interventions
  • The ability to manage a range of tasks including those which fall outside of case management
  • Demonstrated ability to build effective partnerships and work cooperatively with others, including staff, consumers, families, and carers.
  • Demonstrated understanding of professional boundaries.
  • Computer literacy in a Windows environment
