Your Opportunity
Provide structured Alcohol and other drug group programs and individual health and social interventions and support for young people aged 12 -24 years living in the community.
Your key responsibilities will be to:
- Respond to referrals of clients to the service from internal and external support services and conduct formal assessments and collaborative care planning.
- Deliver group-based interventions to young people
- Provide ongoing case management sessions with clients and review progression
- Ensure that all required internal and external client paperwork is completed and copies kept on file
Requirements for Success
- Relevant Qualifications and/or relevant industry experience
- Understanding of issues impacting young people and addiction or experience in that field
- A working knowledge of Case Management and the delivery of group based interventions
- The ability to manage a range of tasks including those which fall outside of case management
- Demonstrated ability to build effective partnerships and work cooperatively with others, including staff, consumers, families, and carers.
- Demonstrated understanding of professional boundaries.
- Computer literacy in a Windows environment
