About the role
The headspace service, delivered in partnership with the National Youth Mental Health Foundation, provides early intervention mental health services to 12-25 year olds in youth friendly spaces. headspace aims to improve access to healthcare and service networks to ensure better care co-ordination by identifying mental ill health earlier, encouraging young people and their families and carers to seek help and support, and providing evidence-based clinical supports. Young people are supported at a crucial time in their lives to get back on track and strengthen their ability to manage their mental health, physical and sexual health concerns, alcohol and other drug (AOD) issues, social and economic needs, and vocational and educational goals through online and phone counselling, information and resources, pathways to work and study, family and carer events, and in-reach services.
We are seeking a highly motivated a Community Mental Health Practitioner – Complex Care Practitioner for a 6 month fixed term, part time opportunity to work in the Greensborough area. You will coordinate care and work collaboratively with a team to enable the best outcomes for young people, families and carers. Acting as a primary point of contact, the Community Mental Health Practitioner – Complex Care Practitioner will support clients to develop skills and build on their own recovery, support with care coordination, to develop strong links with the community, build social connections, identify and provide appropriate supports and resources, offer meaningful referrals to external providers and work alongside clinical partners to ensure an integrated, individualised service response.
Key responsibilities
- Provide comprehensive and a flexible therapeutic support service to young people including assessment, brief intervention and/or longer term counselling, care planning and evaluation.
- Support clients with actioning their recovery plan in a range of areas understanding and managing client’s own mental health.
- Manage intake and assessments, provide counselling and support coordination, deliver brief intervention, access to support services and offer meaningful referrals.
- Identify and implement effective care plans in consultation with young people, their families, significant others and professionals.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Child, Youth & Family Intervention, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
- Proven experience in Youth Work, Community Services, Mental Health, Disability, Social Welfare, Housing, Healthcare or Government sectors is required.
- Experience and expertise in working directly with young people with mental health issues, complex needs and with their families and carers.
- Awareness and understanding of the NDIS is desirable.
- Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
