About the role
Our Primary and Preventative Health team is actively seeking a highly motivated Community Social Worker to join our dynamic and collaborative multidisciplinary team. As a member of our team, you will be responsible for delivering counselling and rural outreach services to our community.
To be considered for this position, you must be a Qualified Social Worker and a member of the AASW. We prefer candidates with previous work experience in a hospital environment and demonstrated skills in counselling, as well as the ability to make sound assessments of treatment needs based on specialist recommendations and advocacy of a client-led process.
Your excellent communication skills and ability to engage with a diverse range of people are essential for success in this role. You must be comfortable working both autonomously and as a member of a team.
The successful candidate will also provide support for patients being discharged from the hospital, ensuring a smooth transition back into their homes and community. This may include monitoring recovery, organising and attending specialist appointments, providing supportive counselling, debriefing, and education relating to their hospital stay, and advocating for patients’ needs with recovery-based organisations.
The position will provide valuable assistance to community members seeking access to Allied Health Services as outpatients, promoting health and wellbeing outcomes through early intervention. The successful applicant will confidently participate in clinical meetings and committees, while taking an active role in developing and delivering group work areas such as chronic disease, postnatal depression, anger management, conflict resolution/mediation, mental health first aid, stress and anxiety, and rural adjustment issues as required.
You will bring:
- Relevant qualifications and registration in Social Work, Psychology or Counselling
- Ability to liaise and consult with health professionals in a multi-disciplinary team
- Effective verbal and written communication skills
- Highly effective interpersonal and team skills
