Community Support Worker New Horizons

JOB DESCRIPTION

We have an exciting Full-time Community Support Worker (Mental Health) opportunity available in our HASI HUB in Southern Highlands.

This position involves a rotating roster, between Monday to Sunday and you will need to be available to work flexible shifts that may include a combination of mornings and evenings.

The Housing and Accommodation Support Initiative (HASI) is a state wide program that supports people with a severe mental illness to live and recover in the community to support Mental Health Recovery.

Services delivered under HASI encompass a wide range of activities that build independence in daily life. Support can include recovery planning, support to live independently, assistance to manage a tenancy, assistance to manage health and emotional wellbeing, as well as support to get involved in your local community.

Based in Bowral, you will report to the Team Leader in our HASI Team.

 

Key responsibilities will include

  • Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals- Regular reviews of progress.
  • Collaboration with Community Mental health staff around clinical goals set with customer.
  • Complete comprehensive risk management plans for customers.
  • Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required.
  • Supporting customers to explore their practical and complex needs.
  • Keeping customer records up to date daily using online systems.
  • The ability to work as part of a Team.
  • Developing and maintaining professional relationships and networks with customers, families, caregivers, and key stakeholders.
  • Seek out innovative ways to enhance the customer journey.

You’ll also need

  • Technical literacy with proven ability to use Microsoft Office and systems for reporting
  • Minimum Cert IV in Mental Health, Cert IV in Mental Health Peer work or equivalent industry experience
  • Availability to work across a rotating roster
  • Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
  • Willingness to obtain an Australian Federal Police Check, WWCC, NDIS Worker Check that meet New Horizons employment criteria prior to commencing with us.
