Your new company
This highly regarded federal government service provider is responsible for the provision of care and support for veteran and defence force members, communities and their families. You will work as part of an Australian wide team, addressing a spectrum of needs and providing support such as health and community care, rehabilitation, counselling, income support and education assistance. They do this via intake and assessments, telephone-based support and case management.

You will be joining an expanding team of passionate and highly professional specialist staff, within a supportive working environment which provides clinical supervision, professional development opportunities and peer and management support.

Your new role
As a Complex Case Manager you will manage a caseload of 25-35 clients, providing specialised case management services to veterans and their families. As their primary point of contact you will provide continual assessment of the client’s care making informed suggestions based upon their ongoing situation. You will oversee and implement a high standard of care for your clients and develop strong stakeholder relationships. You will also liaise with families and third party providers connected with the client to ensure a holistic approach to care is practised.

As part of a Multi-Disciplinary team made up of psychologists, social workers, mental health nurses and occupational therapists you will participate in case conferences to review and collaborate around care. You will be encouraged to work collaboratively with your peers, senior clinicians and receive clinical support on complex cases with a consultant psychiatrist.

What you’ll need to succeed

  • Significant experience working with complex mental health clients
  • Strong Case Management experience, ideally working within mental health
  • Qualifications and or relevant registration with AHPRA as a Psychologist, RN (Mental Health), Occupational therapist or Social Worker (AASW)
  • An understanding of the challenges faced by those who have served in the Australian Defence Force is beneficial
  • Expertise in supporting clients experiencing the impacts of substance abuse, mood and anxiety disorders including PTSD and trauma.
  • Current Police Check, or ability to obtain one
