Key objectives of role:

The Continuing Care Case Manager is to manage the delivery of the Continuing Care Program to ensure that all clients leave South Pacific Private (SPP) with a tailored Continuing Care plan, to co-ordinate, schedule and manage the continuing care of clients attending South Pacific Private.  

This full-time role works closely with the Client & Family Liaison Officer, Day Program Manager, Day Program Therapists, Program Manager, Primary Therapists, the Intake Department, Admin Department and the PR Department.  

Key Responsibilities and Tasks:

  • Manage expectations, attendance and engagement with clients specific to their ongoing Continuing Care / recovery treatment plan  (specific to day programs and the alumni event schedule)
  • Ongoing case management for each client – manage them through their entire Continuing Care delivery plan and continually engage them in the next step of their recovery. Make ongoing appointments to follow through their Continuing Care plan
  • Engage with clients, in order to encourage and facilitate participation in the Family Program
  • Attend clinical handover as relevant to ensure transparency and accuracy
  • Ensure all contact details, activity and engagement are tracked and logged in SPPs Medical Records Database (CareRight) in real-time
  • Contact alumni as per KPIs over the phone on a rolling basis to connect with them with regards to their recommendations for Continuing Care attendance and Continuing Care plan
  • To liaise with the Day Program Therapists and Primary Therapists to discuss and confirm Continuing Care treatment plans and to liaise as per client expectations and realities
  • Meet with each inpatient client on at least 2 occasions during their inpatient admission to discuss and fine-tune Continuing Care treatment planning
  • Ensure day program tracker is accurate at all times
  • Liaise with Admin Department re checking financial viability for program attendance
  • Orient each client in advance of day program attendance with regards to responsibilities, rights, expectations and logistics
  • Provide support to clients on the Mindful Eating Table (MET) as required

Qualifications, experience & personal attributes:

  • A strong aptitude for administration
  • Excellent interpersonal & communication skills, (written and oral)
  • Excellent IT skills
  • Ability to observe and adhere to professional code of ethics and rules of confidentiality
  • Proven ability to work in a multi-disciplinary team environment
  • Ability to be open-minded, flexible, non-judgemental, with an interest in both personal and professional development
  • Self-motivated with an ability to work independently
  • Collaborative planning and organising skills
  • Excellent time management skills
  • Assertive and grounded with the ability to maintain strong boundaries
  • Sound motivational enhancement and motivational interviewing skills
  • Strong & sensitive sales skills
  • To convey a positive, professional image and utilise supervisory feedback to improve performance

Standard requirements:

  • Professional integrity, adherence to a professional code of ethics and confidentiality requirements as per SPP policy and NSQHS requirements
  • Strong interpersonal communication skills and professional boundaries
  • Ability to work in a multidisciplinary team environment effectively, respecting difference and diversity
