About the Program:
The Intensive Community Rehabilitation Treatment Program provides a 5-week holistic treatment and aftercare service for clients who attend the program daily. It incorporates cognitive therapy, behavioural structures, and case planning within a physically and psychologically safe environment, enhancing and encouraging individual motivation for change.

Role Purpose:
The Day Program Case Manager & Group Facilitator will support the day-to-day activities and operations including the creation and maintenance of a safe therapeutic environment. They will work alongside consultants and other specialist staff to lead group-based interventions and assist in the facilitation of psycho-educational groups and the daily client activity schedule. Working under general direction in the application of procedures, methods and guidelines which are well established they will maintain the standards of clinical rigour expected in the management of each client.

The position will also function as a key worker to a case load of clients during and following the program. The key purpose of the role is the implementation and review of an evidenced based program intended to provide behaviour changing interventions to assist individuals to reduce or cease harmful alcohol and other drug use, and to avoid and better manage relapse whilst also assisting participants to establish healthier relationships and lifestyles free of problematic alcohol and drug use.                                                                                                                                                                                                                            

 

Key areas of focus:

  • Case manage, find and engage suitable clients, including conducting complementary client assessments, to develop mutually agreed individual treatment plans (ITPs) and referrals as appropriate.
  • Assess the suitability of clients referred to the program and facilitate the intake and assessment process in collaboration with internal and external service providers.
  • Facilitate program information group sessions and access to accommodation support for clients expressing interest in attending the program.
  • Ensure that clients on a waitlist have Interim Support Plans to help clients avoid and better manage relapse.
  • Work with clients to develop, implement and review their individual treatment goals including aftercare plans.
  • Collaborate with existing service providers and facilitate referrals to other services as indicated.
  • Contribute to the development and implementation of program modules (developmental and life skills groups and activities) ensuring activities are planned in advance, materials are prepared in a timely fashion, groups are conducted, and being actively involved in the organisation and implementation of the schedule.
  • Participate in treatment team reviews, case reviews and other relevant meetings ensuring information and outcomes are current and accurate, and that all client files are up to date and maintained.
  • Support the work of the Team Leader through regular reports on client progress, identifying possible areas of concern as and when they arise.
  • Ensure that the group-based program is planned and delivered in a timely, efficient, and effective way.
  • Collect and record client information including case notes, treatment plans, evaluation data, records of client attendance, etc.
  • Assist with the development of and deliver a manualised cognitive behavioural therapy program to clients inclusive of individual monitoring sessions for a smaller caseload of clients.
  • Facilitate a range of therapeutic and psycho-educational groups, including group interventions for families and significant others.
  • Assist in the appropriate referral and conjoint treatment of clients with specific or specialist needs by liaising with other treatment and community services to achieve the best possible treatment outcomes for clients.

 

 

A week in the role:

If you were here last week, you would have:

  • Presented and assisted with other Day Program facilitators set topics for the day with Clients in a group setting.
  • Attended Clinical Review with the wider Barwon team.
  • Case managed a small group of Clients 1 on 1.
  • Completed case notes.
  • Attended staff, team and client meetings.

 

Key Selection Criteria:

We are looking for an experienced and passionate facilitator to provide behaviour changing interventions to assist individuals to reduce or cease harmful AOD use.     

To be successful in this role, you must…

  • Hold a health or welfare Tertiary/Degree Qualification e.g., in Social Work, Psychology, Nursing or Allied Health Sciences with one years’ experience or an Associate Diploma with relevant experience.
  • Hold a Certificate IV Alcohol & Other Drugs or the AOD competencies, or willingness to complete within first 12 months of employment. *(Refer to Appendix A)
  • Excellent group therapy and case management skills along with the ability to work with difficult and demanding clients.
  • A positive, friendly, and optimistic attitude
  • Comprehensive understanding of alcohol and other drug use, and an understanding of community-based rehabilitation or day programs
  • Demonstrated experience delivering quality outcomes for clients.
  • Ability to show initiative and capacity to work independently, adapting to meet changing organisational needs.
  • Strong organisational and highly developed interpersonal and communication skills (both verbal and written) with a consultative approach to addressing issues.
  • Ability and willingness to contribute and work effectively in a team environment.
