The Role

The DFV Primary Care Outreach Program delivers holistic health care and social support to women and children fleeing domestic and family violence (DFV), residing in crisis accommodation. This multi-agency program ensures that clients receive the right care at the right time by working across service and organisational sectors.

You will lead and coordinate a multi-agency team responsible for providing integrated health care for women and children. While there is minimal direct contact with families, your role is crucial in ensuring the effective management of outreach services, cross-agency collaboration, and the achievement of program objectives, all in line with Coast Shelter’s mission and values.

All about You

• Strong leadership and coordination skills, with experience managing multi-disciplinary teams.

• Excellent relationship-building skills to work with stakeholders across health and social care sectors.

• Experience in program management, especially within a health or social services context.

• Knowledge of DFV and its impacts on families, along with the ability to advocate for their health and wellbeing.

• Proven track record in ensuring compliance and preparing detailed reports on program outcomes.

Key Focus Areas:

• Overseeing the daily operations of the DFV Primary Care Outreach Program to ensure efficient and effective service delivery.

• Coordinating with internal and external stakeholders to deliver integrated care and support.

• Leading multi-agency collaboration to ensure comprehensive care and positive outcomes for families.

• Managing incoming and outgoing referrals and monitoring program effectiveness.

• Supervising team members and ensuring compliance with legal and organisational standards.

Key Responsibilities:

• Oversee day-to-day program operations and ensure seamless service delivery.

• Chair care collaboration meetings to facilitate strong multi-agency partnerships and effective client outcomes.

• Manage referrals for families experiencing DFV, ensuring timely responses and holistic care.

• Monitor program outcomes and provide reflective supervision sessions for the team.

• Build strong relationships with community organizations, healthcare providers, and stakeholders.

• Ensure compliance with all relevant legislation, policies, and procedures, and prepare reports for management and funding bodies.
