About the Role:
In this role you will provide early intervention services through case management and counselling to vulnerable families, children, and young people who display early signs or are at risk of developing mental illness. This role centres around the wellbeing of children within families where there are children under 18 years.
This is a maximum term, part time, 4 days per week till 30th June 2024.
Key Responsibilities (not limited to):
- Ensures individual needs of clients are met by conducting a comprehensive client assessment and providing a family action plan that is developed with the client, is clearly defined and with achievable outcomes
- Ensure the best possible outcome for clients is achieved by advocating for clients, establishing strong network links within local area in order to provide strong external referral pathways and developing strong partnerships
- Ensure the service is well known in the community by developing resources that promote and advertise the service to clients, local forums and other community organisations. This also includes participating in community events that relate to the target group
- Ensure client targets are met by maintaining a client caseload as agreed with the Manager
- Ensure confidentiality of client information is maintained by managing the filing, storage and security of client information
- Ensure documentation meets organisational, legislative and funding requirements by completing and maintaining appropriate data, client documentation and statistical information
About you:
- Relevant tertiary qualifications e.g. Social Work, Social Science, Counselling, Community Services or Behavioural Science
- Previous experience working with children, young people and their families
- Previous experience with counselling children, young people and their families
- Previous experience working with and supporting clients who have been exposed to trauma, violence or neglect
- Excellent written and verbal communication skills
- Valid paid Working with Children’s Check
- Covid19 vaccination is highly recommended
