- Conduct comprehensive client assessments to inform the development of a case plan or appropriate referrals according to the clients identified needs.
- Assess supports and ensure that they are appropriate to meeting client’s needs.
- Refer and assist clients to access appropriate mainstream specialist services and support groups.
- Respond to clients in crisis.
- Facilitate groups as required.
- Recognised qualifications in Psychology, Social Work, Mental Health, Counselling, Childhood Development or other related disciplines.
- Relevant experience in working with children and families with complex needs e.g domestic violence, homelessness, abuse, schooling issues. Whereby service design was inclusive of social, emotional, mental, and physical needs
- C class drivers licence.
- Working with Children Check
- National Police Clearance
- Demonstrated experience in intake, assessment and case management
- Knowledge and experience in early intervention and prevention program services.
- Must be able to work autonomously
