NEWLY CREATED National position available in either Sydney or Melbourne for a compassionate, caring and experienced Health & Wellbeing Manager/Practitioner in the in-home aged care sector

The Role:
Reporting to the Operations Manager and Leadership Team representatives, the Health and Wellbeing Manager is responsible for collaborating with Community Leaders and Health and Wellbeing Coordinators as a trusted advisor to promote community focused care and service.
This is a National role, and we are looking for someone who has a strong understanding of ageing in place and who has outstanding leadership qualities to support a team of Health & Wellbeing Coordinators. You will work closely with the Operations Manager, Senior Leadership Team and Community Leaders to provide professional support and health and wellbeing direction across the Institute.
Other key areas of responsibility will include (but is not limited to):
- Understand, and prepare for, reforming in-home aged care legislation to support Health &Wellbeing Coordinators and sisters, including policy development and provision
- Preparation for implementation of changes that will be introduced following the introduction of in-home care
- Provide education/information relating to ageing to health and wellbeing staff and sisters
- When required, advocate on the sister’s behalf by representing issues to relevant authorities when provision of services and supports are not available
- Resource and support Health & Wellbeing Coordinators to advocate for services and residential accommodation for sisters as needed
- Advising of best practice in relation to apply for and implementing Home Care Packages and support
- Managing all Health & Wellbeing contracts with external partners
- Report regularly to Community Leaders and Institute Leadership about current issues, challenges, and emerging needs across the Institute
To be successful in this role, we are looking for someone who has:
- A relevant tertiary qualification (such as nursing, social work, community services)
- Experience in case management or care coordination in the aged care sector and knowledge and understanding of the in-home aged care sector and government funded services
- Effective communication, listening, interpersonal, influencing and stakeholder management skills
- The ability to manage sensitive inquiries with tact, discretion, and confidentiality
- Strong people management skills

As this is a National role, it is expected that some travel will be required, both intrastate and interstate. The position is a 2 year maximum term contract (with option for review) and a salary commensurate with experience is on offer.
