Health Navigator (0.6 – 1 FTE)

Broome | Salary Range:  $79,665 – $87,829 p.a. (1 FTE)

If you share our values and our passion for delivering the best possible healthcare to clients, you will find a home at Boab Health Services.  Boab Health Services is a not-for-profit, multidisciplinary primary health care service delivering allied health, mental health, and care coordination services throughout the Kimberley region of Western Australia.  We have an opportunity for an experienced Aboriginal Health Worker, Aboriginal Health Practitioner, Nurse, or Allied Health Professional to join our Allied Health team in the West Kimberley. 

 

About the Roles

This important role works with patients, general practitioners, clinic/practice staff and a broad range of other relevant services to provide appropriate multidisciplinary care for people with chronic disease within the West Kimberley region of Western Australia. This includes providing proactive chronic disease education in a way that meets clients’ individual needs to facilitate improved self-management and health outcomes.

 

About You

The right fit for us and our company culture is super important so if you are passionate about delivering the best possible health care we are really looking for you.  You will have a clinical understanding of chronic disease and chronic disease management. You will also have: 

  • A sound understanding and knowledge of general practice, primary health care and health issues in the Kimberley
  • Experience working in Aboriginal communities and working effectively with persons with a chronic disease
  • A Nursing, Allied Health Professional or Aboriginal and Torres Strait Islander Health Practitioner qualification
  • Current AHPRA registration and WA drivers’ Licence
