About the role
The Practice Excellence Coach position sits within the Homelessness Cluster, delivering value-based care through codesign and by developing the capacity and capability for teams to work to the top of their scope of practice and in self-organising ways. The Homelessness Cluster aims to improve the health, wellbeing, housing and social outcomes of people who use alcohol and other drugs and/or those experiencing homelessness in the Inner Northwest of Melbourne.
The Practice Excellence Coach is responsible for the clinical governance and leadership of the Homelessness Services Cluster, ensuring clinicians work to the top of their scope of practice and peer workers are supported to deliver high-quality services in line with cohealth best care framework. The Practice Excellence Coach will ensure staff are supported to deliver programs which are evidence based, client centred, and based on a harm reduction, trauma informed, and recovery orientated framework seeking to empower and educate clients to make informed decisions about their health.
Your daily duties will vary but your main focus will include:
- Provide clinical governance and leadership to support continuous improvement, innovation, and service excellence for Homelessness services at cohealth
- Provide individual and group-based supervision and mentorship to team members to ensure safe, evidenced-based best practice is undertaken by all staff members in the team
- Develop the team’s capacity and capability to work to the top of their scope of practice, including coaching, conflict resolution, group problem solving, team performance monitoring, objective setting and evidenced based practice
- Provide secondary consultation and a point of contact for clinical governance queries as required

About You
- Tertiary qualifications in a relevant health-related discipline (Social Work, Psychology, AOD, or equivalent), and an eligibility to join the relevant professional registration bodies with experience working in community or public health with marginalised populations
- Detailed understanding of evidence-based best practice interventions for consumers with complex needs, including strengths-based frameworks that facilitate consumer choice
- Experience working with highly complex and marginalised clients and their families, including from culturally and linguistically diverse backgrounds
- Ability to work in, and contribute to, an effective, safe and positive service delivery team environment
