Psychosocial Support Worker

MH&W HOPE Team – Melton

  • Melton Mental Health and Wellbeing HOPE is looking for Psychosocial Support Workers to join the team!
  • Unique opportunity to be part of a team delivering brief therapeutic interventions to People at risk of suicide.

  • A person with lived experience of utilising mental health services due to suicide ideation or behaviours, and/or a carer to a person utilising mental health services due to suicide ideation or behaviours is welcome to apply.
  • Clinic based in Melton.

The HOPE team will sit with the broader community mental health team based in Melton. The HOPE initiative has been developed to provide support to individuals discharged from the Emergency Department, medical ward or inpatient units post a suicide attempt or displaying serious suicide ideation, intent or planning in the Melton and parts of Hume (Sunbury) and Brimbank catchment area. HOPE will support clients for a period of 12 weeks, providing assertive and coordinated care post-discharge. Engagement will take place via both outreach and clinic based therapeutic interventions to reduce the risk of suicide re-attempts and deaths by suicide.

  

Your new role 

  • Provide direct assistance to consumers, carers and families
  • Work collaboratively with other community agencies
  • Work collaboratively with the multidisciplinary team to ensure the delivery of a positive and safe experience to at-risk consumers.
  • Communicate effectively with consumers carers and families

 About you  

  • An understanding of crisis intervention and management skills, and ability to escalate concerns if deterioration occurs.
  • Skills in engaging with key welfare, health and other support agencies and working collaboratively to improve consumer’s outcome and recovery.
  • Skills in working with cultural and linguistically diverse communities.
  • A current Victorian Driver’s License, and ongoing ability to use this form of transport.
