Hospital Social Worker – 3 month contract + possible extension CC Medical

JOB DESCRIPTION

 

  • Generalist hospital caseload
  • Less than two hours from Melbourne
  • FREE accommodation available

 
Experienced Social Worker require for three month contract in coastal Victoria location.
 
Join this lovely regional health service, less than two hours from Melbourne. Join an Allied Health team that includes Occupational Therapy, Speech Pathology, Dietetics, Podiatry, Physiotherapy, AOD, Counselling, Social Support Groups, AHAs.
 
The role is to start ASAP. Full time hours, Monday – Friday, 08:00 – 16:30. Three month contract + possible extension.
 
The caseload will cover:
 

  • A mixed caseload, could be required to cover inpatient wards (acute medical/surgical and GEM/Rehab), outpatients and home based services.
  • The role is focused on safe patient discharge following psychosocial assessment; knowledge of My Aged Care essential and knowledge of VCAT & NDIS very desirable
  • This is a Grade 1 or 2 level role and relevant experience is required
  • There are three Social Workers in the team
  • Locum rates $$ and accommodation available
